Function of Position

 

RN Case Manager provides triage of patients in primary care clinic, nursing services in the clinic, including wound care and other nursing management, some outreach, case finding, and clinical case management services in collaboration with clinic staff, to clinic clients of all ages with complex needs.

 

Essential Functions

 

-        Provides nursing services to clients in the clinic.

-        Monitors clinical status of clients.

-        Provides clinic case management services for clients and their families.

-        Provides triage services in clinic.

-        Supervises undergraduate nursing students in clinic.

-        Provides community education in collaboration with clinic staff.

-        Provides or supervises referrals for clients.

-        Provides patient education.

-        Initiates IV treatments when indicated.

-        Institutes emergency procedures when indicated.

-        Trains nursing and medical clinical assistants.

-        Provides lab and phlebotomy services as needed.

 

 

 

Secondary Functions

 

-        Performs other related duties as assigned.

 

Knowledge and Abilities

 

-        Ability to commit and work with underserved populations, especially those without health insurance, those with mental health concerns, those with recognizable resources and those with complex problems.

 

Supervision Given and Received

 

Performs under general supervision.  Determines own methods of performing assigned work.

 

Minimum Qualifications

 

Bachelor's degree required in a service-related field, such as nursing, social work, or psychology; at least one year applicable nursing experience; must be licensed or eligible for licensure as RN in Tennessee.
