Passports - First Time Applicants (Adults 16 years of age or older)

First time applicants must complete an Application for a U.S. Passport (Form DS-11) available at the ETSU Passport Acceptance Office and at www.travel.state.gov.  Note: Application should be completed prior to submitting.

Note: Youths 16 or 17 years old must apply in person with one parent.

Submit the following documents with the completed Form DS-11:

  1. Proof of United States Citizenship (only one of the following is required)
    Note: All birth records must meet U.S. Department of State guidelines.
    1. Original State-issued Birth Certificate with raised, colored, or embossed seal
    2. Certified copy of State-issued birth certificate with raised, colored, or embossed seal
    3. Copies of birth certificates are not acceptable. Birth certificates may be obtained at http://www.cdc.gov/nchs/w2w.htm
      Note: Birth certificate abstracts from California and Texas are not acceptable.  The birth certificate must be a long form.  Birth records issued by a hospital are not accepted
      1. U.S. Citizens born abroad may submit one of the following:
        1. Original Certificate of Naturalization (Adults 18 or older)
        2. Original Form FS 240, Consular Report of Birth Abroad
        3. Original Form DS-1350, Certification of a Report of Birth Abroad
        4. Original Certificate of Citizenship
    4. Applicants may be required to submit the following in addition to proof of citizenship: Original name change documents (marriage license, divorce decree, adoption papers)
  1. Identity Verification
    1. Current state-issued driver's license Note: Out of state driver’s license requires additional ID
    2. Current state-issued ID Card
    3. Current Military ID

Note: If a full adult driver license is not available, multiple forms of ID must be submitted.  Please contact our office for other usable ID.

Minors aged 16 or 17 are not required to present photo ID if the accompanying parent has a valid current ID, such as current adult driver license, valid adult passport, or Military ID.  If the applicant has a valid ID it should be presented at the time of application.

  1. Photocopy of ID front and back (Copies can be made at ETSU Passport Office)
  2. Passport Photo: One recent passport photograph taken within the last six months Photo service provided by ETSU Passport Services for $8.00 tax included. 
  1. Two Required Payments
    1. 1st payment: U.S.Department of State fees (Payable by check or money order only.)
    2. 2nd payment: ETSU fees (Payable by check, money order, cash, credit / debit).
