PDF Format The electronic on-line PDF forms in the table shown above can be completed on your home computer by using Adobe Acrobat Reader. These PDFs can be filled out with input from your keyboard and then emailed to our office. User must have an active internet access as well as an active email account in either Microsoft Outlook, Outlook Express or other comparable programs to properly send these forms by email. | Word Format
The Word format documents will open Microsoft Word and the form may be completed, printed, and saved from Microsoft Word. If you do not have Microsoft Word, you may download the free Viewer for Word 2003 on PCs using Windows XP at Microsoft Office Download Site . Microsoft no longer supports Word Viewers for Word 97, 2000, and XP. With Word Viewer, you will not be able to complete the form from your computer, but you will be able to print it and complete from a typewriter. NOTE ON FILLING OUT DOC FORMS:
