Send all the following to the School of Graduate Studies:
- Application for ETSU Graduate Studies
- Application fee of $25 Domestic; $35 International
- Official Transcripts from all college coursework (No Copies)
- Application Essay (No more than two pages). The letter serves as your introduction to the selection committee and may include relevant background information, statement of goals, summary of related work experience and reasons for selecting ETSU.
- Verification Statement or Declaration of Intent to Graduate (No Copies-available from DPD director)
- Supervised Practice Program Application (ADA)
- Waiver and Recommendation Form
- Send all application materials to the following address:
