HOFFMANN SIGN
Hoffmann sign is flexion and adduction of the thumb in
response to sudden flexing of the distal joint of the
ipsilateral middle finger; other fingers may also
flex.This patient also had thumb clonus in response to
the test (see video).
Although occasionally found in normal
persons, a positive Hoffmann sign usually indicates
the presence of pyramidal track disease - in this
case the result of compression of the cervical cord by
subluxed vertebrae and herniated discs (see CT
scan).
