Domestic Applicant Information
Note: Applicants who wish to enter the Doctor of Public Health, Ph.D. in Environmental Health Sciences,
Master of Public Health, or M.S. in Environmental Health programs programs should make initial
applications through SOPHAS.org. All other graduate
applicants should apply through GoldLink.
A complete application will include the on-line electronic application form, university
requirements, and departmental/program requirements:
Step 1: Complete the on-line application form and submit it electronically
(including application fee).
Step 2: Collect the additional materials listed below and mail in one large
envelope to The School of Graduate Studies - Box 70720 - Johnson City, TN 37614:
A. University requirements:
- Non refundable application fee of $35.00 made payable to East Tennessee State University.
You may pay by check, money order or credit card (MasterCard or VISA only)--do not
include cash.
- An official transcript from every college or university where you have previously
taken classes, even if courses from one school appear as transfer credit on the
transcript of another. Transcripts must be placed in official, sealed envelopes;
please request that the Office of the Registrar at the issuing institution sign
and/or place an official stamp across the seal. Unopened sealed transcripts should
be included in your application packet. NOTE: We will verify in-house all ETSU academic
work for students who have earned a degree or taken classes at ETSU.
- A brief, typewritten essay of approximately 150 - 300 words, which conforms to accepted
standards of good written English. The essay should address your areas of interest
in graduate school, especially your proposed field of study, abilities or strengths
you bring to the program, and your career objectives. Other topics might include
your undergraduate record, including major and minor, honors you have received,
and teaching or work experience. If you have a weak undergraduate background that
does not denote potential for success in graduate school, indicate clearly why you
feel you can successfully pursue graduate work.
- Residency Inquiry Form,
if claiming Tennessee residency. Available for download at this site. Print the
form and complete by typing or hand-printing information legibly. Be certain to
sign the document, mail to the above address or fax (423) 439-5624.
B. Departmental/Program requirements:
See Degree Program Admission
Requirements and/or the
Graduate Catalog to determine whether the program of your choice requires
GRE scores, letters of recommendation, etc.
- If available, you may send photocopies of GRE
or GMAT test score reports as required by the
program to which you are applying. HOWEVER, official scores must also be sent directly
to us from the Educational Testing Service (ETS).
- Letters of recommendation, if required by your program (see
Degree Program Admission Requirements and/or the
Graduate Catalog) may be completed through the Online Letters of Recomendation System.
If internet is not available, a Manual Recommendation Form is available for download and should be mailed directly
to your references. Each reference should complete the form, place it in a sealed
envelope, sign across the seal, and return the envelope directly to you. These unopened,
signed, sealed envelopes should be included in the packet.
- Additional requirements, if specified by your chosen program.
Step 3:Items which must be sent directly to the School of Graduate Studies
separate from the packet:
- Official GRE or GMAT scores, if required by your program, must be sent directly
to the School of Graduate Studies from ETS--even if photocopies were included in
your packet.
- Some institutions may refuse to send official transcripts directly to you, in which
case, they may be sent directly to the attention of your
Graduate Program Specialist at School of Graduate Studies. Your application
cannot be fully processed until all official transcripts have been received.
