The beginning of the ETD Review Process. All of your committee members will sign this form to release your manuscript for review. Your manuscript will not be reviewed until this form is on file with the Graduate School.
Notifies the Graduate School of the level of access you will allow Sherrod Library and Digital Commons at ETSU once your ETD is approved. Your ETD will not be approved until this form is turned in.
Notifies ProQuest of the level of access you will allow them once your ETD is approved. The Graduate School will send a copy of your ETD to ProQuest microfilm production and archiving along with any order forms you fill out. Your ETD will not be approved until this form is turned in.
