Departments hosting visiting scholars must complete this form
requesting International Programs and Services to issue the scholar
the form DS-2019 used in applying for the J-1 visa.
If visiting scholars are eligible for an extension of stay, two
forms must be completed: one by the hosting department and the
other by the visiting scholar himself or herself. Extensions
request should be submitted to International Programs and Services
at least 2 weeks before the current DS-2019 expiration date.
Eligible visiting scholars planning to transfer to another J-1
program must submit to International Programs and Services a SEVIS
transfer out request.
