Deadline Extended Until November 30!

SUBJECT:  Employee Giving Campaign

        As you probably well know, the Employee Giving Campaign for this year scheduled to end this Friday is only at the 75% mark leaving approximately $20,000 yet to be raised.

         I certainly realize that these are very tough times, and it is difficult to meet our own personal obligations let alone give beyond that.  Please realize, however, that there are many around us who are struggling even more in these most difficult times, and the need is greater than ever.  Please consider donating to the campaign if your heart as well as pocketbook will allow.  We will extend the campaign through November 30.

Pledge Form Instructions

For more information about the charitable organizations, please open the Campaign Brochure (and the United Way of Dyer Co. Addition). 
The Pledge Form can be found by clicking on the bottom of any page in the Brochure, pages 20-24.

1.       Fill out all 5 pages of the Pledge Form.

 If completing the form online, all 5 pages will be completed automatically as you type the first form.

2.       Once completed, please sign ALL 5 pages.

3.        A. If you are NOT using payroll deduction: Write a check to the designated agency you are giving to (if more than one agency, please write more than  one check).
B.  If you ARE using payroll deduction: Be SURE to write your E number in the ID# field, DO NOT write your SSN.

4.        Keep the "Donor" Form.

5.       Submit the remaining 4 forms (and your check(s), if appropriate) to Office of Human Resources, Box 70564.
