Directions for Acrobat Reader (.pdf) format
The electronic on-line .pdf forms in the table can be completed
at your computer using Adobe Acrobat Reader which is
a free downloadable software from the internet. This software can
be downloaded by clicking on the following icon ---> Version 5.0 This icon downloads Acrobat Reader 5.0 for
Microsoft® Windows® 95 OSR 2.0, Windows 98, Windows
Millennium, Windows NT®* 4.0 with Service Pack 5 or 6, Windows 2000, or Windows
XP.
This icon downloads Acrobat Reader 4.05 for Windows 95. After downloading this application file, you must then run
this file to install Adobe Acrobat Reader. The download is not
complete until you run this file and accept the Acrobat Reader use agreement.
After doing this, any .pdf file, including these forms, should automatically
open for you. For more detailed directions of working with Adobe Acrobat, please
click here. NOTE: These .pdf forms can be completed and printed using
your personal computer and Adobe Acrobat Reader. The .pdf forms
cannot
be completed using ANY word processing software such as WordPerfect
or Microsoft Word. The .pdf forms
must be completed using
Acrobat Reader.

When you click on the above icon, you will
be downloading a file named ar500enu.exe. When you click,
you will be asked where you want to save the file. You can choose
any
directory or folder on your hard drive or you can save the file to the
desktop. The file takes a few minutes to download depending
on your internet connection.
These .pdf forms can be downloaded to your computer and filled out with input from your keyboard. The current version of Acrobat Reader 5 allows .pdf files to be saved to your computer hard drive.

Working With .pdf Forms - When using Adobe Acrobat Reader software, following these steps will allow you to make the most efficient use of the software.
Step 1 - Before you fill out the form, save the .pdf form to your hard drive (works with Acrobat Reader 5.0 or higher only; if you are using a version lower than Reader 5.0, please skip this step.) You may be prompted by your browser to save when you first click on the link. Or, clicking on the link may automatically open Acrobat Reader and the form. If this happens, use the Save button to save the file to your hard drive.
Step 2- The saved version of the file is the copy you want to use to fill out the form. This will allow you to save a copy of the completed form. If you are unable to save the form to your hard drive (or don't want to), you can still complete the form and print the data.
NOTE: Completing the form without saving to your hard drive may appear to 'save' the data 'out on the Internet.' If you return to the link using your browser, you may notice all of your data is still there. This data is stored ONLY on your hard drive and nowhere else. The user cannot accidentally save a completed form to the Internet. The system will not allow this to happen. If you wish to complete another form, delete the old data as you enter new data.

Directions for Word Format
The Word format documents will open Microsoft Word and the form may be completed, printed, and saved from Microsoft Word. If you have problems with using the Word (.doc) format, please try using the Acrobat (.pdf) format.
If you do not have Microsoft Word, you may download the free Viewer for Word 97/2000 on PCs using Windows 95/98 or Windows NT by logging on at:

With Word Viewer, you will not be able to complete the form from your computer, but you will be able to print it and complete from a typewriter.

ETSU SECURITY INFORMATION
East Tennessee State University makes available to prospective employees the ETSU Security Information Report. The report is published annually and includes the following information:
- campus policy regarding the reporting of criminal activity
- campus policy concerning facility safety and access
- campus policy concerning law enforcement
- information regarding safety and crime prevention programs
- campus policy regarding the sale, possession and use of alcohol and illegal drugs
- information regarding drug and alcohol education programs
- campus policy regarding sexual assault programs to prevent sex offenses and procedures to follow when sex offenses occur
- description of procedures for preparing annual disclosure of crime statistics
- campus policy allowing disclosure of crimes on a voluntary, confidential basis
- campus policy concerning crimes disclosed to a pastoral or mental health counselor
- campus policy concerning the recording of student criminal activity at off-campus locations
- timely warning procedure (campus crime alerts)
- crime statistics for the three most recent calendar years
