replicating the community cancer control forum

INTENT

  • To replicate the Community Cancer Control in Appalachia Forum in the designated Appalachian regions of states

  • To improve awareness of special Appalachian cancer concerns

  • To identify common interests between Appalachian communities and state CCCCs and promote potential collaboration opportunities

ELIGIBLE APPLICANTS

  • State Comprehensive Cancer Control Coalitions (CCCCs) as recognized by the CDC

  • Regional coalitions of State Comprehensive Cancer Control Coalitions as recognized by the CDC are eligible with support from the statewide CCCC.

  • Appalachian community organizations which demonstrate evidence support and involvement from state CCCC for the project

 SIZE AND NUMBER OF GRANTS

  • Successful applicants will be awarded funds up to $5,000

  • Total of 10 awards

 REQUIREMENTS

  • Submit agenda for proposed forum that addresses Key Elements

  • Commitment letters from primary partners

  • Budget (including details and justification)

  • Explain process to identify and involve “best practices” in cancer control that are operating within the state’s Appalachian region

  • Use Give-Get Grid to contributions and potential benefits to collaboration
