Prior to submitting the eligibility checklist in step one, please obtain the following! Taxable income amount for the most recent tax return.

Step One

1. Go to the eligibility checklist, complete all the questions and submit the form electronically.

Once your eligibility checklist has been reviewed by the ETSU McNair staff you will be contacted via e-mail and instructed to proceed to step two

Step Two

Once your eligibility checklist has been reviewed by the ETSU McNair staff you will be contacted via e-mail and instructed to proceed to step two.

1. Go to the application form, complete all questions and submit your application electronically.

2. Fill out the online recommendation form waiver and submit electronically.

3. Complete and submit a type-written Statement of Purpose

4. Send the Statement of Purpose by e-mail to Dinah DeFord

5. Submit a Current Transcript to the address shown below (Unofficial transcript is acceptable)

6. Most recently filed tax return( IRS Form 1040) (used to determine program eligibility based on income).

Step Three

Below is the Recommendation form in .PDF format.

• Have two evaluators complete the form and return the document to Dinah DeFord.

• At least one evaluator must be able to evaluate academic course work.

• Recommendations should be forwarded via U.S. Mail or fax. 

• Only ETSU faculty may forward recommendations via e-mail
