Step One

1. Go to the eligibility checklist, complete all the questions and submit the form electronically.

Once your submission has been reviewed you will be contacted via e-mail and instructed to proceed to step two

Step Two

1. Go to the application form, complete all questions and submit your application electronically.

Step one must be submitted and approved before the application form can be reviewed

2. Fill out the online recommendation form waiver

3. Faculty members that you request recommendations from may also wish to see a copy of this waiver before submitting the recommendation so please print a copy for your own records.

4. Use a word processor and complete a Statement of Purpose

5. Send the Statement of Purpose by e-mail to Dinah DeFord

6. Current Transcript (Unofficial transcript is acceptable)

7. Most recently filed tax return( IRS Form 1040) (used to determine program eligibility).

Step Three

Below is the Recommendation form in .PDF format. Have your evaluators complete the form and return the document to Dinah DeFord. Recommendations should be forwarded via U.S. Mail or fax.  Only ETSU faculty may forward recommendations via e-mail
