Please submit copies of your documentation, not originals, to the Office of Academic Programs and Student Services, Nicks Hall, room 230. (Copies cannot be made for you. A copier is available in the Student Lounge located in Room 276 Nicks Hall)

OSHA training - After you have read the Infection Control/Hazardous Material Training for Nursing Majors Introduction and completed the Power Point Presentation, go to the D2L site CON Office of Student Services, then click on Assessments and take the OSHA Post-Training Quiz. If you are not able to go to the D2L site, please print out the quiz below, complete it and return to the Offices of Academic Programs and Student Services, Room 230, Nicks Hall. You can also fax a copy to them at 423-439-4522. Please be sure to put your name on the quiz.
