Immunization Exemptions

Tennessee High School Graduates after June 1, 2016

These students do not have to provide documentation for MMR or Varicella vaccines. You are automatically EXEMPT from these vaccines only for ETSU general registration purposes. No further action is needed for these two vaccines in order to register for full-time classes.

In-State Incoming Students to ETSU

New incoming students to ETSU who have received one MMR (measles, mumps, and rubella) and one Varicella (chicken pox) vaccination may complete the One Semester Immunization Exemption Form and return it to the Registrar's Office so they may be able to register for a full time class load. (Available at the Registrar's Office or complete the form ONLINE.

Out-of-State Incoming Student Exemption

New OUT-OF-STATE incoming students to ETSU who have received only one MMR (measles, mumps, and rubella) and one Varicella (chicken pox) vaccination may complete the One Semester Immunization Exemption Form and return it to the Registrar's Office so they will be able to register for a full-time class load (12 hours undergraduate and 9 hours graduate). Available at the Registrar's Office or complete the form ONLINE.

Online Students

Students who are taking exclusive online and distance-learning courses only are exempt from the MMR and Varicella requirements, unless you are living on campus; however you must complete the Online Only Immunization Exemption form each semester and file it with the Registrar's Office. Available at the Registrar's Office or complete the form ONLINE.

Medical Exemptions

Students who are allergic or have medical conditions preventing them from receiving these vaccinations may complete a Medical Waiver for Immunizations form accompanied with signed medical documentation and submitted to the Student/University Health Services for exemption.

Pregnant and Breastfeeding Students

Pregnant or Breastfeeding students may complete the Medical Waiver for Immunizations form accompanied with signed medical documentation and submitted to the Student/University Health Services for temporary exemption. 

Religious Exemption:

In accordance with the State and ETSU policy, you may submit a Notarized Religious Exemption Form to Student Health Services.
