The Purchasing Office is in the process of converting printable forms to electronic on-line forms. Please bear with us as we implement this conversion. Forms which can be completed online are so noted in their description.

Instructions for forms which cannot be completed online. These forms must be downloaded and printed off your computer. They can then be completed by hand or on a typewriter.
Electronic On-line Forms
.PDF FORMAT
The electronic on-line .pdf forms in the table can be completed at your computer using Adobe Acrobat Reader which is a free downloadable
software from the internet. This software can be downloaded by clicking on the following icon --->

When you click on the version 5.0 icon, you will be downloading a file named ar500enu.exe. When you click, you will be asked where you want
to save the file. You can choose any directory or folder on your hard drive or you can save the file to the desktop. The file takes a few
minutes to download depending on your internet connection.
After downloading this application file, you must then run this file to install Adobe Acrobat Reader. The download is not complete until
you run this file and accept the Acrobat Reader use agreement. After doing this, any .pdf file, including these forms, should automatically open
for you.
NOTE: These .pdf forms can be completed and printed using your personal computer and Adobe Acrobat Reader. The .pdf forms cannot
be completed using ANY word processing software such as WordPerfect or Microsoft Word. The .pdf forms must be completed using
Acrobat Reader.
These .pdf forms can be downloaded to your computer and filled out with input from your keyboard. The current version of Acrobat Reader 5
allows .pdf files to be saved to your computer hard drive.
NOTES:
You may need to use the TAB key to position the cursor in the first data field.
Use the TAB key to move forward from blank to blank and SHIFT+TAB to move backwards through the form.
Some entries are check boxes and require the mouse be used to check (or uncheck) the box.
Upon completing a page of entries, the TAB won't take you to the next page. Use the mouse to click the scroll arrows to move
to next page, then continue entering data into the blanks as before. The mouse is always used to move to the next page.

WORD FORMAT
The Word format documents will open Microsoft Word and the form may be completed, printed, and saved from Microsoft Word. When
completing the form in Microsoft Word, should a formatting, length of fill-in blank, or other problems occur, click the "View" menu on the
toolbar, go to "Toolbars" and click "Forms". When the "Forms" toolbar appears, click the picture of the lock to unlock the form. The form
may then be manipulated as needed. This action returns the document to a regular Word document. If you continue to have problems with
using the Word (.doc) format, please try using the Acrobat (.pdf) format.

With Word Viewer, you will not be able to complete the form from your computer, but you will be able to print it and complete from a
typewriter.

EXCEL FORMAT
The Excel format documents will open Microsoft Excel and the form may be completed, printed, and saved from Microsoft Excel. The form
is locked and password protected to allow entry into only the appropriate cells. This allows automatic calculations to be performed on the
data entered.
If you do not have Microsoft Excel, you may download the free Viewer for Excel 97/2000 on PCs using Windows 95/98 or Windows NT by

With Excel Viewer, you will not be able to complete the form from your computer, but you will be able to print it and complete from a
typewriter.
