ATTENTION:

In order to be able to fill out, save and submit the online RDC application, you must use Adobe Reader or Acrobat 8.0 or greater.
You can download the latest Adobe Reader from http://get.adobe.com/reader/

If you use any other PDF viewer, such as Apple Preview or Google Chrome PDF viewer, etc., you will likely not be able to submit the PDF form - clicking the submit button will do nothing. Additionally, required fields, character limits, and formatting, etc. may not work correctly.  Make sure you tell anyone entering information into the RDC application to use Adobe Reader or Acrobat to fill out the form, otherwise they may run into any number of problems.
