.pdf Format
The electronic on-line .pdf forms in the table can be completed at your
computer using Adobe Acrobat Reader (version 7 is best) which is a free downloadable
software from the internet. This software can be downloaded by clicking
on the following icon --->
For this version of Adobe Acrobat Reader to properly install, it is important that you uninstall all previous versions of Acrobat Reader by going to "Control Panel" choosing the option to "Add/Remove Programs" and then picking the previous versions to uninstall.
These forms can be filled out on-line and submitted via e-mail by pressing the submit button at the top of the form. These .pdf forms can also be downloaded to your computer and filled out with input from your keyboard. The file cannot be saved to your computer but can be printed.

 | You will also need to create an electronic signature within Adobe Reader before trying to fill-out and submit one of the forms on-line. To do this, start Adobe Reader and go to the "Documents" tab and then choose the "Manage Digital ID's" option and then choose "Select My Digital ID File" and finally choose "New Digital ID". At this point fill in the information and follow the prompts. When completed you will have a digital ID that can be used to sign any electronic document... |

Upon completing a page of entries, the TAB won't take you to the next page. Use the mouse to click the scroll arrows to move to next page, then continue entering data into the blanks as before. The mouse is always used to move to the next page... |

| When you click on the above icon, you will be downloading a file named AdbeRdr60_enu_full.exe. When you click, you will be asked where you want to save the file. You can choose any directory or folder on your hard drive or you can save the file to the desktop. The multi-megabyte file takes a few minutes to download depending on your internet connection. After downloading AdbeRdr60_enu_full.exe, you must then run this file to install Adobe Acrobat Reader 6.0. The download is not complete until you run this file and accept the Acrobat Reader use agreement. After doing this, any .pdf file, including these forms, should automatically open for you. For more detailed directions of working with Adobe Acrobat, please click here. NOTE: These .pdf forms can be completed and printed using your personal computer and Adobe Acrobat Reader. The .pdf forms cannot be completed using ANY word processing software such as WordPerfect or Microsoft Word. The .pdf forms must be completed using Acrobat Reader. |
Word Format
The Word format documents will open Microsoft Word and the form may be completed, printed, and saved from Microsoft Word. If you do not have Microsoft Word, you may download the free Viewer for Word 97/2000 or XP on PCs using Windows 95/98 or Windows 2000, NT, ME or XP by logging on at:

With Word Viewer, you will not be able to complete the form from your computer, but you will be able to print it and complete from a typewriter.
