On-line Electronic Forms
These forms can be completed at your computer using Adobe Acrobat Reader. If you do not already have Adobe Acrobat Reader installed on your computer, the software can be downloaded by clicking on the Adobe Acrobat Reader icon on the web page.
To determine if you already have Adobe Acrobat Reader installed on your computer:
Click on one of the forms listed on the forms web page. If you already have Acrobat Reader, you will be able to view (or open) the form. If you do not have Acrobat Reader, you will get some other kind of error window or message.

Click on the Adobe
Acrobat Reader icon on the web page.
1. When you click on the Adobe Acrobat Reader icon, you will be downloading a file named ar405eng.exe. When you click, you will be asked where you want to save the file. Choose to save the file on your Desktop. This will create an icon on your Windows Desktop that will be used later to install the program. The 5.6 megabyte file takes about 30 seconds to download depending on your PC.
2. After downloading ar405eng.exe, minimize (or close) your browser window. You should have a new icon on your Windows Desktop. You can then double-click on this icon to install Adobe Acrobat Reader 4.0. This will take just a few minutes to install and requires the user to provide input by clicking on the NEXT button when appropriate.
3. When the installation finishes, you will be prompted to re-boot the machine. This is not a necessity and can be done later. From this point forward, Adobe Acrobat Reader is on your machine and you will know
longer have to install it.
4. The first time you try to access one of the forms (or any .pdf file), you will have to accept the terms of the license agreement.
Following your one-time acceptance of the license agreement, the computer will automatically open Adobe Acrobat Reader
7.0 and display your file. From this point on, any .pdf file, including these forms, should automatically open for you.

To Save and Use An Electronic Form:

NOTE: These forms can be completed and printed using your personal computer and Adobe Acrobat Reader. They cannot be completed using ANY word processing software such as WordPerfect or Microsoft Word. The forms must be completed using Acrobat Reader. |
While on the internet, click on the desired University Center document.
After clicking, either:
a) the computer will automatically open Adobe Acrobat Reader and you will be able to immediately view the form, or;
b) you will be prompted to either OPEN or SAVE the file.
** If prompted as in #2, choose open the file. Once you can see the form, click on FILE, and then click on SAVE AS.
1. The computer will then ask you where you want to save the file. The file can be saved in any location you want, but be sure to remember where you saved it. The file name can be changed but do not change the file extension (.pdf).
2. After saving the .pdf file on your computer, you can then open the file by going to either MY COMPUTER, WINDOWS EXPLORER, or by opening Adobe Acrobat Reader and then opening the desired file.
3. Once opened in Acrobat Reader, the form can be completed with input from you keyboard. Using the mouse, click in the desired fields (blanks) and enter the appropriate information. Treat the file as you would any word processing document. It can be printed when finished. Be sure to save on your computer or on floppy disk.
NOTES (about above electronic form):

Use the TAB key to move forward from blank to blank and SHIFT+TAB to move backwards through the form...

Some entries are check boxes and require the mouse be used to check (or uncheck) the box...

Upon completing a page of entries, the TAB won't take you to the next page. Use the mouse to click the scroll arrows to move to next page, then continue entering data into the blanks as before.
The mouse is always used to move to the next page...

After the file has been completed, save the file with a different name than the original.
This will allow you to keep a copy of the blank form stored on your computer for future use...
