Informed Consent

We would like to use the results from the Tennessee Intervention for Pregnant Smokers in order to improve smoking cessation services, help more women become smoke free, and educate healthcare providers on the most effective ways to help their patients stop smoking. If you choose to participate in this aspect of the program, we must have your informed consent.  Please know that your personal information will always be confidential and never released.  We hope you will consider this and choose to contribute to healthcare knowledge!

You can view a copy of the Informed Consent Document here.
