Regional Conference for Complex Patients
There is a growing need for appropriate solutions to care for complex patients. Research shows that 1% of all patients use 20% of all healthcare resources/costs. This is true for primary care patients seen here in this region. Area medical providers and healthcare systems are scrambling to deploy Patient Health Managers and implement other collaborative practice models. Their objective is to improve the health among patients with complex physical and social needs while decreasing overuse/misuse of healthcare options.
