WE CARE! WE WANT TO ADDRESS CONCERNS!
Quillen College of Medicine cares about its students, faculty and staff. We have created several forms below so that we can recognize important exciting events and engagement. We also want to support our students during times of growth and learning with forms to recognize concerns about our students. Please help us stay in touch!
CARE
I have something exciting to share! Information and Praise Form (etsu.edu)
Tell us about something exciting that has happened to you personally or professionally!
We want to hear about these events and let the Quillen family know about it too! (with
your permission)

CONCERN
Professionalism Concern - The Quillen College of Medicine is dedicated to helping future physicians develop
and maintain professional behaviors. Professionalism may encompass communications
between patients and providers; appropriate attire/appearance; reliability, motivation,
and responsibility; interpersonal relationships; ability to accept instruction and
feedback; integrity. The Quillen College of Medicine professionalism policy and procedures
support the ongoing development of learners and provide a framework for reporting
unprofessional behaviors and correcting any deficiencies.
Through modeling and upholding professionalism, the College supports ETSU’s values statement: people come first, are treated with dignity and respect, and are encouraged to achieve their full potential; relationships are built on honesty, integrity, and trust; diversity of people and thought is respected; excellence is achieved through teamwork, leadership, creativity, and a strong work ethic; efficiency is achieved through wise use of human and financial resources; and commitment to intellectual achievement is embraced.
The Student Professionalism Report is designed to allow faculty, staff, or students to document an incident or trend of student unprofessional or unethical behavior. It is also used to measure institutional success in accomplishing the Quillen College of Medicine’s Professionalism Institutional Educational Objective.
The professionalism policy can be found here.

CARE Form
A Care form is an online form that anyone associated with Quillen can complete to
call attention to concerns regarding a student. It should not be used in the case of EMERGENCY or if immediate attention is needed. In an Emergency, please call 911. If an urgent situation arises, some options are highlighted here: Get Help If You Need It (etsu.edu). Any concern about a student’s welfare may be reported. This includes, medical, mental
health, academic, and physical health issues.
CARE reports can be confidential, but you are encouraged to provide information in case additional information is needed. The reporter’s identity will not be disclosed to the student of concern. A limited number of Student Affairs staff have access to the form.
CARE reports are reviewed daily by Student Affairs staff. If it is determined that further action is needed, the student will be contacted and interviewed in person to assure safety. Depending on the severity of the concern, additional steps will be taken based on Quillen COM policy to include Fitness for Duty.
