Practice


Practice Network Clinics

  • Practice network clinics had 41,006 patient encounters in AY2017
  • Approximately $1.2 million in uncompensated care was delivered
  • Received $1 million from HRSA for construction of the new Johnson City Day Center which serves the homeless population that opened Jan. 2018
  • CHC Recognized as a National Quality Leader and a Health Center Leader plus PCMH recognition. This indicates that we are in the Top 30% of al health centers.
  • CHC accepted to the Health Center Medical-Legal Partnership Learning Network
  • Clinic patients are: 38% self-pay; 24% Medicaid; 9% Medicare; and 29% insured
  • CHC accepted to participate in the Team-Based Care Learning Collaborative supported by CHC, Inc. and the Weitzman Institute
  • CHC accepted to participate in the Post-Graduate Residency Learning Collaborative supported by CHC, Inc. and the Weitzman Institute
