Required Information Packet

Please carefully review these documents prior to completing the required observation hours:

Application Form - Can be completed on-line.

Clinical Instructor Contact Sheet - Call and make your appointment for observation up to 6 months prior to interview.

Directions for Clinical Site Observation - All of the hospital which participate in observation are listed.

Clinical Observation Form – take this form with you to your observation experiences. 

Admission to the professional curriculum under the Baccalaureate Allied Health degree is a competitive process and is separate from the student’s admission to ETSU.  A limited number of students are admitted annually.  Each applicant is evaluated on the basis of academic record, clinical observation, and personal interview.


Candidate Admission Eligibility

To become a candidate for admission, the student must meet the minimum requirements for admission.  The applicant is required to:

  • maintain a college GPA of 2.5 or better on a 4.0 scale.

  • complete prerequisite general core curriculum and other allied health core courses as required and earn a grade of C- or better.

  • complete the general core curriculum as indicated on the program advisement sheet during the freshman and sophomore years.  Only six credits of general and allied health core curriculum from the freshman and sophomore years may be carried into the program of study.

  • complete a minimum of two hours of observation at each of the program’s clinical affiliates, with exception of the VA Medical Center, within 6 months of the interview.  A total of 20 hours are highly recommended.

  • be advised by the program director prior to the interview.

  • interview with program faculty during the months of March or April during the year intended to enter the program. 


Program Acceptance

To be accepted to the program, the student must:

  • maintain a college GPA of 2.5 or better on a 4.0 scale.

  • complete prerequisite general core curriculum and other allied health core courses as required and earn a grade of C- or better.

  • attend the clinical in-service meeting.

  • provide verification of current CPR certification.  CPR certification is required to be kept current during the radiography program.

  • complete and submit the dosimeter request form, malpractice insurance form, background check, and the health verification form.  Information must be kept current during the radiography program.

  • attend the program orientation.

  • meet specific health and/or essential functions


Alternate Status

Alternates may be accepted into the program up to and including the second week of the fall semester of the junior year.
