Registration Information

All registration forms, audition tapes and payment in full must be postmarked by May 30, 2014. A separate registration form must be completed for each student, auditor, and teacher participant attending the Institute. Please feel free to photocopy registration forms as needed. You may fax your registration if you are paying by credit card and if no audition tape is required. All registration fees are quoted and payable in U.S. dollars. Checks should be payable to East Tennessee State University.

Please review the registration form thoroughly before returning to make sure you have provided complete information. Incomplete forms, registrations without full payment of fees, and applications requiring audition(s) that are not accompanied by audition tapes will not be processed. You are encouraged to make a copy of your completed registration form to keep for your records.

Cancellation/Refund Policy

Your total Institute registration fee minus an $80 processing fee and your on-campus housing fee, if applicable, will be refunded if it becomes necessary for you to cancel your registration on or before May 30, 2014. No refunds will be given for cancellations received after May 30, 2014.

Persons with disabilities who require special assistance or seating at this event are requested to call the Suzuki Institute at (423) 439-4270 for details. Persons who require sign language interpreters should contact the Suzuki Institute at least thirty days prior to the event in order to solicit qualified interpreters.
