Job Description:

SUMMARY OF POSITION
A child & family specialist is responsible to the program director for carrying out the functions of family assessment, family support, and intervention to enable children to have permanence in the most family like setting as quickly as possible. The child & family specialist, in conjunction with the family and DCS, develops, monitors, and assures the implementation of the individualized service plan and plan of care.

JOB QUALIFICATIONS/REQUIREMENTS
1. Must possess a bachelor's degree in social work or a related social service field. A Master of Science degree in social work or related social service field is preferred. Licensure or ability to pursue licensure is also preferred.
2. Experience working with families and their children in custody or at risk of being in custody is preferred.
3. Must be committed to empowerment of families and be willing to work in a high demand work situation which requires flexibility and innovativeness.
4. Must be knowledgeable of Holston Home's Family model and demonstrate competency in applying this knowledge to working and joining with families to encourage the swiftest path to permanence for children.
5. Must demonstrate cultural competence by being aware and sensitive to the family's ethnic, cultural, spiritual, and community norms.
6. Must meet clients and families in their homes with flexible appointment times based on families' needs, including evenings and weekends.
7. Must be competent in the delivery of the services of assigned program (foster care, residential, etc) and willing to use a range of interventions & modality
8. Must be committed to the team approach among biological family members, resource families, other helping professionals and other family support persons
9. Must possess excellent written and oral communication skills.
10. Must possess or obtain a valid Tennessee driver's license and a reliable means of insured transportation.
11. Must have the physical health and capacity to demonstrate regular job attendance and, when necessary, physically manage client's aggressive or self-injurious behavior consistent with TCI and agency requirements.
12. Must be sighted and able to travel freely throughout the community in facilities that are not handicap accessible.
13. The Child & family specialist position involves a risk of exposure to communicable diseases; therefore, the employee must be familiar with/and practice Universal Health Precautions.
14. Must meet annual in service training requirements.
15. Must subscribe to the Agency's Mission Statement and Organizational Values.

ESSENTIAL JOB RESPONSIBILITIES
1. Join with and engage biological and any resource family as full partners in treatment process to empower families to take control of their own lives, while instilling a sense of hope;
2. Serve as advocate for families and their children;
3. Participate in the admissions process for each child/family accepted onto caseload;
4. Continually assess family and child's strengths, challenges and barriers to permanence;
5. Provide skill building, counseling and/or therapy as appropriate to level of service for the youth and the staff's credentials. These interventions will be provided for biological families, other caregivers or resource families;
6. Provide 24 hours/7 day on call support to the case for handling crisis situations as dictated by program description. In some instances this will require face-to-face support;
7. Attend weekly supervisory sessions & treatment teams;
8. Conduct Child and family team meetings to develop and assess formalized service plans for each child in care; the child and family specialist should facilitate the family & child identifying their goals for treatment;
9. Maintain up-to-date case records in compliance with all licensing, accreditation & monitoring standards and timelines;
10. Develop aftercare plans and provide follow-up services as outlined in agency procedure;
11. Maintain frequent & regular open written & oral contact with the family regarding the child's needs;
12. Schedule and plan goals for home visits with family members and resources;
13. Discuss medication needs with the child and family and obtain consents from the child and family;
14. Attend all child and family team meetings scheduled for each client;
15. Assure timely follow-up on professional and team recommendations;
16. Assess client safety issues in conjunctions with the family and/or caregivers, and report safety concerns immediately to Director;
17. File serious incident reports consistent with agency procedures;
18. Follow agency procedures for ensuring worker's safety while in the field;
19. Develop a safety plan in conjunction with the family to be implemented during crisis situations;
20. Serve as liaison between the biological and any resource family agency, custodial worker, school personnel, extended family, foster family and community resources;
21. Represent the agency's position in court proceedings and to DCS prior to court;
22. Facilitate and attend all treatment related intakes (i.e. mental health/psychiatric, alcohol and drug treatment);
23. Secure all relevant treatment documentation from family, prior placements, or hospitalizations;
24. Transport clients and family members as needed, using agency or personal vehicles.
25. Inform director, in a timely way, with all relevant case or staff issues.
26. In the event that the youth being served are in residential treatment, the CFS will serve as resource for residential regarding treatment issues.

SECONDARY JOB RESPONSIBILITIES
1. Participate in appropriate staff development activities;
2. Communicate identified needs for on-going in-service training to the Director;
3. Communicate child placement moves to intake;
4. Plan and lead appropriate treatment and educational group activities;
5. Develop and assess objective measures for recording behavioral progress of each client;
6. Keep up-to-date statistical reports, as assigned;
7. Assure orientation of new admissions according to outlined procedure;
8. Inform and consult with all team members and professional staff regarding the needs and service plan goals for each family;
9. Maintain documentation for Quality Improvement goals;
10. Communicate relevant case specific information with child and family team;
11. Be available for other meetings that impact the child and family;
12. Carry out other duties as assigned by the director.
