NOTE: Completed forms MUST be submitted and received before availabilities and /or arrangements can be discussed / considered. The review process requires submission of all documents at least eight (8) weeks prior to the requested rotation dates. ALL visiting student applications MUST be processed through the Office of Academic Affairs.
