Financial Aid Satisfactory Academic Progress Academic Plan Form
(Student should complete the upper section and meet with an ETSU Academic Advisor to complete the second section. Then, the student should submit the form to the ETSU Office of Financial Aid. After a Financial Aid Administrator has noted the terms of the Academic Plan on the form, the student will then sign the Academic Plan.)
