The Word format documents will open Microsoft Word and the form may be completed, printed, and saved from Microsoft Word.  When completing the form in Microsoft Word, should a formatting, length of fill-in blank, or other problems occur, click the "View" menu on the toolbar, go to "Toolbars" and click "Forms".  When the "Forms" toolbar appears, click the picture of the lock to unlock the form.  The form may then be manipulated as needed.  This action returns the document to a regular Word document.   If you continue to have problems with using the Word (.doc) format, please try using the Acrobat (.pdf) format.
