Washington County School Based Health Centers 

The ETSU College of Nursing School-Based Clinics at David Crockett, Daniel Boone, and Jonesborough Elementary schools are supported by Washington County Schools and ETSU. Like your own doctor’s office the school based clinics offer primary care services by licensed nurse practitioners and these services are billed to your insurance. If your child does not have insurance there will be a sliding scale based on income. Those that are uninsured and do not meet the sliding scale guidelines will be billed $25.00 for an office visit.

Many students may be exempt from charges due to income. For those that have insurance, they will be billed the co-pay. Please think of the school-based clinics as a safe and convenient place for your children to be seen for minor illnesses and urgent care needs that require the expertise of a nurse practitioner at school. Your child will continue to be referred back to their primary care practitioner as needed.

For insurance and TennCare billing, we will need a copy of the child’s insurance card. The current Consent Form for students also has a place for insurance information on page 2 of that form.  If you want your child to be seen in the nurse practitioner clinics they will need the Consent Form filled out in full and returned when student starts back to school. A copy of the forms will also be available on-line at the ETSU College of Nursing website ( http://www.etsu.edu/nursing/practice/sites ), and on the Washington County Schools website.

Important Physical Exam Info

 

  • DOT Physicals for bus drivers- Please contact the Johnson City Community Health Center.
  • Students and WCS Staff- If you have not received a Physical Exam from your primary care provider within the past 12 months, we will be happy to perform a Physical Exam and file your insurance.

All checks should be written out to ETSU College of Nursing.
