Immunizations Required Under the Tennessee State Law
The State of Tennessee requires that all students enrolled in state
colleges and universities meet certain immunization requirements
prior to their initial enrollment in the university or
college. These requirements include:
1. Proof that the Student has received
(2) MMR (Measles, Mumps, and Rubella)
Shots or Physician signed documentation of immunity.
2. Proof that the Student has received (
2) Varicella (Chicken Pox) Shots or
Physician signed documentation of immunity.
3. A
Signed Waiver form acknowledging that
the student is aware of the risk associated with Hepatitis B
and Meningitis Infections on a college campus.
Exemptions
:
1) New incoming students to ETSU who have received one MMR
(measles, mumps, rubella) and one Varicella (chicken pox)
vaccination may complete the One Semester Immunization
Exemption Form and return it to the Registrar's Office so they
may be able to register for a full time class load.

2) Pregnant or Breastfeeding students may complete the Medical
Waiver for Immunizations form accompanied with signed medical
documentation and submitted to the Student/University Health
Services for temporary exemption.
3) Students who are allergic or have medical conditions
preventing them from receiving these vaccinations may complete
the Medical Waiver for Immunizations form accompanied with
signed medical documentation and submitted to the
Student/University Health Services for exemption.
4) Students who are taking exclusive online and
distance-learning courses only are exempt from the Varicella
requirements, but must complete the Online Only Immunization
Exemption form each semester and file it with the Registrar's
Office.

MMR Requirement
It is Tennessee State Law that all
full-time students provide proof that they have
received Two MMR Shots. The following exemptions apply to
this State Law:
- Students born prior to 1957.
- Students who graduated from a public or private high school in Tennessee in May 1999 or at any point thereafter.
- Students who attended a public or private school in Tennessee for grades Kindergarten through twelve for any period of time on or after July 1, 2001.
- Students who provide a signed written statement that their religious tenets and practices prohibit their receipt of any type of immunizations. Religious Exemption Form
- Students with written documentation from a physician certifying that they are allergic to the MMR vaccine. Medical Waiver for Immunizations Form
- Students with written documentation from a physician attesting that they have a diagnosed case of Measles, Mumps, or Rubella.
- Students with written documentation of their laboratory confirmed immunity to measles, mumps, or rubella.
- *Students who graduated from a public or private Tennessee high school between May 1979 and December 1998 are not required to provide proof of immunization with the first dose of the MMR vaccination.
- Students who are classified as active duty military personnel and provide proof of their active duty status.
- Students who were previously enrolled as a full-time student
at a TBR college or university for at least one full semester
after August 1, 2007.
Hepatitis B/Meningitis Waiver Requirement
All students must provide a signed waiver regarding Hepatitis B and Meningococcal disease prior to enrollment in the University. There is no exemption to this waiver form.
Students under the age of 18, must provide a signed waiver that is signed by either a parent or legal guardian.
Students 18 and over, will be able to complete this form online electronically when they attempt to register for classes the first time.
NEW VARICELLA REQUIREMENTS EFFECTIVE
JULY 1, 2011
If you were born on or after January 1, 1980, to be a
full-time enrolled student, you must provide proof of Two doses
of the Varicella vaccine or physician signed documentation of a
history of the disease. If you cannot provide this documentation,
you may provide proof of the results of a positive titer from a
medical laboratory or a provider's office for this disease. If
you are taking on-line classes only, you are exempt from this
requirement, but you must complete the Registrar's "On-Line Only
Waiver Form" each semester.
