Immunization Requirements

The State of Tennessee requires that all students enrolled in state colleges and universities meet certain immunization requirements prior to their initial enrollment in the university or college.  These requirements include:

  1. Proof that the Student has received Two MMR (Measles, Mumps, and Rubella) Shots
  2. A signed waiver acknowledging that the student is aware of the risk associated with Hepatitis B and Meningitis Infections on a college campus.

MMR Requirement
It is Tennessee State Law that all full-time students provide proof that they have received Two MMR Shots.  The following exemptions apply to this State Law:

  • Students born prior to 1957.
  • Students who graduated from a public or private high school in Tennessee in May 1999 or at any point thereafter.
  • Students who attended a public or private school in Tennessee for grades Kindergarten through twelve for any period of time on or after July 1, 2001.
  • Students who provide a signed written statement that their religious tenets and practices prohibit their receipt of any type of immunizations. Religious Exemption Form
  • Students with written documentation from a physician certifying that they are allergic to the MMR vaccine.
  • Students with written documentation from a physician attesting that they have a diagnosed case of Measles, Mumps, or Rubella.
  • Students with written documentation of their laboratory confirmed immunity to measles, mumps, or rubella.
  • *Students who graduated from a public or private Tennessee high school between May 1979 and December 1998 are not required to provide proof of immunization with the first dose of the MMR vaccination.
  • Students who are classified as active duty military personnel and provide proof of their active duty status.
  • Students who were previously enrolled as a full-time student at a TBR college or university for at least one full semester after August 1, 2007.

Note: Please be advised that there is No Grace Period for compliance with this State Law.

Hepatitis B/Meningitis Waiver Requirement
All students must provide a signed waiver regarding Hepatitis B and Meningococcal disease prior to enrollment in the University.

Those students under the age of 18, must provide a signed waiver that is signed by either a parent or legal guardian.


NOTE: Please note that all students who are 18 years of age or older at the time that they enroll with the university for the first time will be prompted to complete this form online electronically when they register for the first time.
