When finished, print, sign and return this form to the Office of the Registrar, Graduation Section, 102B Burgin Dossett Hall or mail to:

Major and Minor Sheets
All Major and Minor sheets must be printed, completed, and submitted to the Graduation office before the Intent to Graduate form will be processed. See your college for specific instructions.
Undergraduate Course Substitutions
Advisors may fill in, print and submit the undergraduate course substitution form to the graduation office with appropriate signatures for recording in the student file.
Absentia
If you cannot attend the graduation ceremony, complete the absentia form.
Change Catalog Year
Student/Advisor may fill in, print and submit the change of catalog to the graduation office for recording in the student file.
