JULY 1, 2011

If you were born on or after January 1, 1980,  to be a full-time enrolled student, you must provide proof of Two doses of the Varicella vaccine or physician signed documentation of a history of the disease. If you cannot provide this documentation, you may provide proof of the results of a positive titer from a medical laboratory or a provider's office for this disease. If you are taking on-line classes only, you are exempt from this requirement, but you must complete the Registrar's "On-Line Only Waiver Form" each semester.
