Equipment Reservation
Complete the form below, and submit it to equipment room staff at least 24 hours prior to checkout.
Equipment User Agreement
Bring completed form with ETSU ID and valid photo ID to Warf-Pickel, room 113.
Personal Appearance Release
This form authorizes the use of a subject's name, likeness, image, voice and interview.
- Gear Checkout Procedure

1. Students must submit their signed Equipment Use Agreement, scan their ID in the equipment room and be properly entered into the checkout system before they will be permitted to check out the equipment.
2. A reservation form signed by the class instructor must be submitted at least 24 hours before equipment pick-up time for the reservation to be valid.
3. Check out a temporary parking permit to use the loading and unloading zone when you are checking equipment in or out.

Equipment is always due back the day after it is checked out by 10 a.m. Late returns will merit a late fee of $50.00 per day.
