Most Recent Version of the TRAQ (2021)
This updated version of the TRAQ is recommended for clinical use.  The sections "Tracking Health Issues" and "Talking with Providers" were expanded to be more sensitive to the full range of transition skills and abilities. This newest version represents the third iteration of the TRAQ. Additionally, two additional questions were added at the end to assess a youth's motivation and confidence. By adding these questions, the youth's self-efficacy can be determined by the self-reported scale.  The final section "Managing Daily Activities" was removed in this version. Find this version here.

First Version of the TRAQ (2011)
This is the first version of the TRAQ. Motivation and confidence are not measured, and the sections "Tracking Health Issues" and "Talking with Providers" are not expanded. Additionally, there is a fifth section titled "Managing Daily Activities." Find this version here

TRAQ is available to providers or families for download in Word format in either English, Spanish or French.

TRAQ using Excel
TRAQ item scores can be entered into this Microsoft Excel template. The spreadsheet will automatically calculate the domain and total scores. The first row of data is a “sample” that illustrates how data are entered. The blacked out area for the domain scores and total scores row becomes readable as the relevant item responses are entered into a specific row.

As you use these resources, we ask you to provide us with your contact information. This will enable us to provide you with additional information in the future including updates, new resources (such as an online scoring system), and methodological publications on TRAQ 5.0. We are also planning a newsletter that would include publications using TRAQ and other news. A more formal registration requiring a password is also being planned.
