Immunization

Compliance with the following physical and immunization standards is required prior to your initial enrollment as a medical student.

• I. PHYSICAL EXAMINATION

  You must obtain a physical examination dated after your acceptance to medical school. The AAMC Immunization Form must be used and uploaded to myRecordTracker. However, If you have had a physical examination within the last six months, you may have your Physician's office complete the form. Please note that a statement from your physician stating that he/she examined you and found you to be in good health is not sufficient.

• II. PROOF OF IMMUNITY FOR SPECIFIED INFECTIOUS DISEASES

  Measles (Rubeola), Mumps, Rubella (German measles), Varicella (chicken pox)

  To prove immunity, you must have the following information:

  A. Documentation of adequate immunization 

  Measles Mumps and Rubella
  Two doses of combined live MMR vaccine or alternately two doses of live Measles virus vaccine, one dose of live Rubella vaccine, and one dose of live Mumps virus vaccine.

  Varicella
  Two doses of live Varicella vaccine or a blood serum titer test with results of the test with a numerical value that indicates immunity.

  
  -or-

  Laboratory evidence of immunity: date of the titer, results of the test with a numerical value for the presence of antibodies that indicates immunity and name/location of the laboratory.

  -or-

  Documentation of physician-diagnosed illness.

  If you cannot prove immunity through the above means, immunization with MMR vaccine, and/or Varicella vaccine must occur.

  Hepatitis B

  
  To prove immunity, you MUST have documentation that you have completed the Hepatitis B vaccine series, which consists of 3 inoculations taken over a 4- 6 month period.

  -and-

  A blood serum titer test (quantitative Hepatitis B surface antibody) that demonstrates adequate antibody levels.

  Documentation of antibody testing must include:

  The date of the titer.

  Results of the test: this MUST include a numerical value for the presence of  antibodies that indicates immunity and name/location of the laboratory.

  
  
  Students failing to achieve immunity following completion of the vaccine series will undergo a second series of vaccinations and follow-up blood serum testing.

• III. OTHER REQUIREMENTS

  Diphtheria-Pertussis-Tetanus (DPT)

  
  You must provide documentation that you completed the initial (i.e., childhood) series of vaccinations. This series consists of three doses of combined vaccine and should have been received in childhood.  Students unable to provide such documentation will be required to receive immunization with Tdap. 

  
  Tetanus-Diphtheria-Pertussis (Tdap)

  
  You must provide documentation that you have received at least one Adult booster vaccine within the past 5 years.
  
  

  Tuberculosis (TB)

  
  You must obtain a test for TB (by the Mantoux method) dated after your acceptance to medical school and forward documentation of the test results to the Office of Student Affairs. The results must include the date, the results and the location of the facility where the results were read.

  If the results of your TB test are positive, or if you have a history of being PPD-positive, you will be required to obtain a chest x-ray and forward a report of the findings including the status of any INH therapy. Annual Influenza vaccine is required of all QCOM students. Immunization for the novel coronovirus (COVID-19) is required of all QCOM students. 

• IV. SUGGESTED VACCINATIONS

  Meningococcal vaccine is required for students residing in a dormitory on ETSU campus. Refusal of the vaccine must be documented by signing the attached waiver form.

Insurance

• I. Disability Insurance

  All students are required to purchase and maintain disability income insurance as a condition of enrollment at the College of Medicine. The fee for the AMA-sponsored student disability income insurance is currently $48.03. This amount is subject to change. The annual fee will be included on your Fall fee bill (dependent coverage not available). The complete brochure can be obtained from Student Support Services. • II. Health Insurance

  All students are required to purchase and maintain health insurance as a condition of enrollment at Quillen. This requirement must be satisfied by purchasing insurance and providing proof that you have coverage. A good resource is Farm Bureau. We have done extensive research and, although we do not sponsor any program or vendor, the policies available from Farm Bureau seem reasonable. Some reviewing the options suggest that the Core Choice Plan might be appropriate for most students. You currently receive $2750 in financial aid to apply to the cost of a health insurance premium. This amount is consistent with AAMC guidelines. Select your coverage wisely so that it is both cost effective and sufficient for your needs.

  Please provide evidence of health insurance by completing the Insurance Declaration Form on an annual basis and uploading to MyRecordTracker.   

• III. Professional Liability Insurance

  Professional liability insurance is provided for students by the Office of Student Affairs through State Volunteer Mutual Insurance. Information regarding limits of liability can be obtained from that office at 423-439-2019.

Health Services, Counseling & Mental Health

• ETSU Student Health Services

  Enrolled students are able to use the Student Health Clinic for their health care. Clinicians will make referrals for services not available. Students requiring care for chronic, on-going illness will be referred to an appropriate provider. Some laboratory procedures are performed in the clinic.

  University Health Center ETSU Psychology Clinic

   

• PARC House

  The Professional and Academic Resource Center (PARC) provides confidential counseling services to medical students, residents and their families.

• ETSU Counseling Center

  The Counseling Center provides mental health, career, educational and consultative services to ETSU students.

• Committee Supporting Student Health (CSSH)

  Committee Supporting Student Health (CSSH) program assists students and residents who have been identified as suffering from a treatable dysfunction such as chemical dependency.
