status?
Only persons notified by the Office of Human Resources are required to provide documentation.

What types of documentation will be accepted as proof of my vaccination?
Any of the following are acceptable forms of documentation: a copy of the record of immunization from a health care provider or pharmacy; a copy of the COVID-19 Vaccination Record Card; a copy of medical records documenting the vaccination; a copy of immunization records from a public health or State immunization information system; or a copy of any other official documentation verifying vaccination with information on the vaccine name, date(s) of administration, and the name of the health care professional or clinic site administering vaccine.

Why are ETSU employees required to provide this documentation?
On January 13, 2022, the Supreme Court of the United States (SCOTUS) upheld the Centers for Medicare and Medicaid Services Mandate. As a result, individuals who work for a Medicare or Medicaid certified provider are required to either be vaccinated against COVID-19 or file for a medical or religious exemption.

What if I wish to request a medical or religious exemption from the COVID-19 vaccination mandate based on my disability, medical condition, or personally held religious beliefs?
In lieu of proof of vaccination, you will need to provide a completed copy of the Request for Medical Exemption from the COVID-19 Vaccination Requirement form or the Request for Religious Exemption from the COVID-19 Vaccination Requirement form to Human Resources, through the electronic or in-person options listed below.

How will I know when my documentation has been reviewed and accepted?
If you submitted your documentation electronically, you may check the real-time status of your submission by clicking the "Forms History" link located under the "My Forms" drop-down menu in the upper right portion after logging into Dynamic Forms. This form will be listed under the title "COVID-19 - Employee Vaccination Status" and all submissions will initially show a status of "Multi-Pending", which indicates that your form is currently awaiting processing by Human Resources. Requests for religious or medical exemption will also display a status of "Pending", which indicates that your documentation is currently under review for approval. A status of "Processed" indicates that your submission has been fully reviewed and completed all associated steps as necessary to record your status. Submissions of only vaccination documentation will simply display a status of "Multi-Pending" or "Processed" and will not show a "Pending" status. Failure to provide this documentation in a timely manner and/or comply with ETSU's
emergency COVID-19 Vaccination requirement or federal law may result in disciplinary
action.
ETSU encourages all affected employees to promptly provide proof of their COVID-19 vaccination or provide their request for a medical or religious exemption to the Office of Human Resources through one of the options listed below.

The COVID-19 Employee Vaccination Status Dynamic Form is confidential and your submission will only be accessible by a limited number of approved staff in the Office of Human Resources as necessary for processing. However, documentation submitted regarding requests for medical or religious exemptions may be reviewed by additional staff in the Offices of Disability Services, University Compliance, or other offices as necessary to fully process your form. You will not be required to enter any routing information because your form will automatically route directly to Human Resources. You will receive an automated confirmation email upon the completion of your form by Human Resources and you may check the status of your form anytime by accessing your Forms History in Dynamic Forms.
The usage of a smartphone or tablet is strongly recommended if you simply need to submit proof of your vaccination because most mobile devices will conveniently prompt you to simply take a picture of your vaccination record card as you are filling out this form.

The usage of a desktop computer or laptop with an available scanner, including but not limited to your ETSU assigned computer or laptop, is strongly recommended if you need to submit other types vaccination documentation aside from simply a vaccination record card or if you need to upload/attach the Request for Medical Exemption from the COVID-19 Vaccination Requirement or the Request for Religious Exemption from the COVID-19 Vaccination Requirement.
