Dear Parent/ Legal Guardian and Student of University High School:

We hope you are doing well during this time. We are reaching out about your student’s potential participation in a research study. Our high school has agreed to participate in this local study on the role of school closures and the COVID-19 pandemic on adolescent anxiety. This research will be conducted using a secure online survey (which takes about 5-10 minutes) through ETSU REDCap with questions for your high schooler to answer. It has also been approved by East Tennessee State University’s Institutional Review Board (IRB).

 ALL responses will be anonymous and unlinked to the consent and assent information. More details on the study are included in the survey.

The investigator is University School student Amisha Agrawal (agrawala@etsu.edu) and her research advisor is Ms. Meg Robinson (robinsommk1@etsu.edu). Please feel free to reach out to them or ETSU IRB with any questions.

There is an informed consent and assent section prior to the survey itself. The link to this survey is below and will be open from February 15- February 28.

Please complete the survey at your leisure. We hope that the results of this research will give us a better understanding of the mental toll or lack thereof that this health crisis has taken on our student body.
