Fall sports will soon be starting up at University School, this is a reminder to please
turn in . Physical Forms , Cardiac Arrest Forms, and Concussion Forms . TSSAA policy states that physicals dated before 4/15/20 expired on 6/30/20. Physicals
with a date of 4/15/20 or later will be valid until 6/30/21. All of last school years
concussion and cardiac forms expired 6/30/20. This year, due to COVID, parents may scan and submit the required forms to this secure ETSU Dropbox Link. Prior to scanning the completed forms, please be sure to include all initials and
signatures where applicable. Athletes cannot try out for any sport prior to submitting
the completed forms to the University School Athletic Department. Anyone desiring
to try out for any sport must submit the completed forms prior to the tryout date(s).
Parents should keep the original hard copies of the forms for their own records.
