Kudos Recognition
Recognize a resident, fellow, student, faculty or staff member who has gone above and beyond expectations. Honorees may be featured in the newsletter and entered into a monthly drawing.
General Suggestion Box
Submit a general suggesiton for the Department of Internal Medicine.
Report a Professionalism Concern
Internal Medicine is committed to supporting professional behavior among physicians, faculty and staff. Professionalism includes:
- Communication with patients and colleagues
- Appropriate attire and appearance
- Reliability, motivation and responsibility
- Interpersonal relationships
- Ability to accept instruction and feedback
- Integrity
The Professionalism Report allows faculty, staff, fellows, residents and students to document concerns related to unprofessional or unethical behavior. Reports also help measure progress toward professionalism milestones.
Every effort will be made to address concerns while maintaining confidentiality for all parties involved.
Care Form
The Care Form allows anyone associated with Quillen to report concerns about a resident or fellow, including:
- Medical concerns
- Mental health concerns
- Academic performance issues
- Physical health concerns
Emergency Support
Do not use the Care Form for emergencies.
- Call 911 for immediate assistance

What Happens After You Submit
- Reports are reviewed daily
- A limited number of faculty and staff have access
- Reports may be submitted confidentially
- The trainee may be contacted for follow-up
- Additional actions may be taken based on Quillen College of Medicine policy, including Fitness for Duty
Confidentiality
You are encouraged to provide your contact information in case follow-up is needed. However, your identity will not be disclosed to the individual of concern.
