This form is only for Undergraduate students. Remember to view Degree Works before submitting the DW Exception Form.
If also submitting a Major Change or Catalog Change Form, please check Degree Works to ensure the change has been made before submitting any exceptions.
Note exception practices:
- Lower division courses should not be substituted for upper division courses
- Exceptions should not be requested if the requirement is fulfilled
- Follow departmental procedures for approval of substitutions
- If an exception needs to be made for a large group of students, please contact graduation@etsu.edu before submitting the forms
